
In-Home Health Care Policy
1. Purpose
The purpose of this policy is to establish guidelines for providing high-quality, safe, and ethical in-home health care services to clients in need of medical or personal assistance within their homes.
2. Scope
This policy applies to all caregivers, nurses, home health aides, and other employees or contractors providing in-home health care services under the organization.
3. Eligibility for Services
Clients eligible for in-home health care services must meet one or more of the following criteria:
Require assistance with daily living activities (e.g., bathing, dressing, meal preparation).
Have a medical condition that necessitates home-based nursing care.
Need post-hospitalization rehabilitation or palliative care.
Have a physician’s referral for home health care services.
4. Caregiver Responsibilities
All caregivers must:
Provide compassionate, respectful, and professional care to clients.
Follow individualized care plans developed by health care professionals.
Adhere to all safety protocols to prevent injuries or harm.
Maintain accurate and confidential health records.
Report any concerns regarding a client’s condition to a supervisor immediately.
5. Health & Safety Protocols
Caregivers must follow universal infection control procedures, including hand hygiene and the use of personal protective equipment (PPE) when necessary.
Emergency procedures must be followed in case of medical emergencies, falls, or other safety incidents.
Home assessments must be conducted to ensure a safe environment for both caregivers and clients.
6. Confidentiality & Privacy
All client information must be handled in accordance with HIPAA regulations.
Caregivers must not disclose any personal or medical information without client consent, except as required by law.
7. Training & Compliance
All employees must complete training in patient care, infection control, first aid, and emergency response before providing services.
Ongoing training and evaluations will be conducted to ensure compliance with industry standards and company policies.
All caregivers must supply their own patient care insurance for medical malpractice; Connect4Care is not liable for any patient injury.
8. Service Termination
In-home health care services may be terminated if:
The client no longer meets eligibility criteria.
The care required exceeds the scope of in-home services.
The client or family engages in behavior that endangers the caregiver.
The client refuses care or is non-compliant with treatment plans.
9. Reporting & Grievances
Clients and caregivers can report concerns or grievances to the agency’s designated representative.
Reports will be investigated promptly, and appropriate action will be taken to resolve issues.
10. Policy Review & Updates
This policy will be reviewed annually and updated as necessary to reflect regulatory changes and best practices in in-home health care.
Connect4Care will provide a list of professional caregivers that have passed a criminal background check and are licensed in the care they provide.
Connect4Care will not be held liable in any way for the actions, conduct, or services provided by the caregivers listed in its directory. The responsibility for vetting, contracting, and supervising caregivers rests solely with the clients or their representatives.
11. Confidentiality & Privacy
All client information must be handled in accordance with HIPAA regulations.
Caregivers must not disclose any personal or medical information without client consent, except as required by law.
